CMS-1500 HAS BEEN Replaced

 

     The proposed 1500 claim form is NOW in effect as of January 2007.  HOWEVER, Medi-Cal will not accept the new form until MARCH 26TH 2007.


     The National Uniform Claim Committee (NUCC) announced on June 10, 2005 the opening of a 45-day public comment period, ending July 25, 2005, for the proposed NUCC Health Insurance Claim Form to replace the current 1500 version.  The proposed version contains a number of enhancements that have been researched over the past year.

 

     The NUCC is conducting a final survey to better understand the timelines and transition issues surrounding the implementation of the proposed changes.

 

Development of the Proposed 1500 Professional Claim Form:


     For over a year, the NUCC has been examining the CMS-1500 Claim Form in preparation for the implementation of the National Provider Identifier (NPI). The NUCC recently made recommendations regarding the placement of the NPI on the CMS-1500 paper Claim Form. The NUCC recommendations include minor changes to some of the box headings in the 1500 claim form and instructions for the appropriate placement of the NPI.
